Planning for Incapacity
Our team will gather key information about your needs, then provide all details to your attorney to prepare legal planning documents naming MonarchCare should you or a loved one become incapacitated.
Once named in those documents, inform your key contacts to contact MonarchCare whenever you need us. We will then meet with you to collect information about your healthcare choices and create a file in advance of any service needs. We encourage regular check ins with us to ensure we have the most up to date information in our files
Planning for Advocacy
The "Care Management Reservation" program provides you with a local, stand-by advocate, available at a moment’s notice for future care management needs. With this service there is no need to locate a care manager, schedule an assessment, get to know one another and set up a file; all this is taken care of ahead of time so we can start advocating for you when you need it most.
The "Care Management Reservation" program is most useful should you or your loved one be hospitalized in an emergency, have a medical issue requiring immediate advocacy, a family member out of town wanting to have a local advocate check in on a loved one, etc. and need our services to begin as soon as possible.
Our Mission is support vulnerable individuals who need help with life management due to age, disability or family circumstance.
